What is Scoliosis and how to treat it.

Scoliosis is when the spine does not grow straight in a child (see picture A). One of the important reasons to bring your child in for his/her well visit every year is so that your child’s provider can screen for this.

Some scoliosis is due to conditions such as cerebral palsy. It can also be seen in a more temporary manner if a patient has a back spasm, herniated disk etc. Sometimes children can be born with it, which we check for at birth.

What Do Scoliosis Exams Involve?

On exam providers will check the child’s leg length, because one leg being longer than the other can cause the appearance of scoliosis. We then have the child stand with his/her feet together and lined up then bend to touch their toes (see picture B).

It is important to make sure that the child is not leaning one way or the other and his/her legs are straight. We then look at the patient’s back to see if one side is higher than the other. We also look for shoulder unevenness, the scapula (shoulder bone) being more noticeable on one side, waist unevenness, and small chest or bumps by the spine

How It Is Treated

If a patient has a concerning exam, the provider will order an X-ray. Typically if there is more than a 20-degree curve on X-ray, we will refer a patient to an orthopedist (bone doctor).

The earlier a curve is found, the earlier treatment can be started in hopes to keep the curve from getting worse. When a patient has a 25–to-30-degree or more curve on X-ray, or scoliosis that is rapidly progressing, the patient will be treated by spinal bracing.

If a patient has a curve greater than 45 to 50 degrees, or their scoliosis is worsening despite bracing, they will likely require surgery. If a patient has a curve more than 75 to 80 degrees, the lungs and heart can be affected. This is why we try to intervene as early as possible.
