New patients can request an appointment online or call their preferred PAK Pediatrics location to schedule.
After you submit the online appointment request form, a member of our team will contact you within 1-2 business days to discuss your child’s needs and help schedule the appropriate appointment.
The online appointment request form is for new patients only. Submitting a request does not confirm an appointment. A member of our team will contact you to finalize scheduling.
Scheduling for Established Patients
Online scheduling for established patients is coming soon!
Until then, please call your preferred PAK Pediatrics location to schedule an appointment. Our team will be happy to help you find the right appointment, provider, and location for your child.
After scheduling, all established patients must confirm their appointment and complete the pre-visit registration before their visit. Please look for an email and text message containing your secure registration link.
Established patients should not use the new patient appointment request form.

When a minor illness can’t wait, EZ Care makes it easier to get your child the care they need—without a scheduled appointment.
EZ Care offers same-day walk-in care for established PAK Pediatrics patients over 6 months old with common minor illnesses and concerns, including:
- Coughs and congestion
- Sore throats
- Ear pain
- Fevers
- Rashes
- Other minor illnesses
Your child will receive convenient, high-quality care from the PAK Pediatrics team you know and trust.
